Transitional Care

TRANSITIONAL CARE offers an extended care option to meet the individual needs of patients during the transition phase of illness or recovery.

Patients may be able to participate in the program if their medical needs cannot be met at home but the condition no longer requires a stay in an acute care setting. We provide continued recovery services designed to meet individual patient needs.

OUR CARING STAFF will provide individualized treatment to assist the patient in reaching their maximum rehabilitation potential. The program is designed to help patients regain their independence and bring them back to their prior level of health so they can return home safely.

PATIENTS CAN REMAIN in the program as long as they have therapy and relevant rehabilitation goals to meet. Once goals are reached, most patients are able to return home. Some patients may need to explore additional care options.

To qualify, you must have a skilled medical need. Medicare guidelines and your insurance coverage determine eligibility. Medicare patients are required to have been admitted for three overnights in an acute care setting before moving to a transitional care unit. Commercial insurances require prior authorization. Our team reviews each patient’s case and manages the authorization process for you.

You can transfer to JCH Transitional Care from another hospital.

IF YOU’RE IN THE HOSPITAL and advised that you need to have additional skilled nursing care or rehabilitation, you can request a referral to Jersey Community Hospital Transitional Care. A member of our team will review your needs and eligibility with you and the referring facility.

You may need care after:

  • Joint replacement (knee, hip, etc.)
  • Rehabilitation after surgical procedures
  • Recovering from a major accident
  • Rehabilitation after a stroke or heart attack
  • IV medication therapy
  • Receiving wound care
  • What are the benefits?

Patients are more at ease when the care is custom, hospital- based with familiar physicians and there is more time for recuperation. Transitional care creates a natural transition back to independent living, where family and friends can visit providing emotional support necessary for recovery.
