Community Survey

Jersey Community Hospital (JCH) is currently undertaking a Community Health Needs Assessment (CHNA).  We would appreciate your help with this important survey.  By conducting a Community Health Needs Assessment every three years, our hospital and partners can put together a plan to meet the community’s needs.  These needs are being identified by key stakeholders – healthcare professionals, local health department staff, city and county leaders, business owners and YOU!

Each community is unique and has its own healthcare needs.  These may include:

  • Obesity, weight loss and diabetes prevention, or
  • Alcohol abuse, or
  • Lack of access to specialty health care

There are many factors that you think are important that are not on this list.  We want your feedback.

How will we use this information?

Once data is collected, the health needs are prioritized to focus on the community's greatest needs. JCH has administrators, caregivers, and partners who work together to support existing community-based health programs and services. New programs may be implemented to help each community meet its specific needs. 

As JCH moves forward this year, we want to focus our efforts on issues and initiatives that our communities feel are important. Please help us with this goal.

Instructions

We want to know how you view our community, so we are inviting you to participate in a survey about our community health needs. Your opinions are important. This questionnaire will take approximately 10 to 15 minutes to complete.

All of your individual responses are confidential. You will not be identified in any way and no one will contact you or follow up with you. We will use the results of the surveys to improve our understanding of health needs in the community.
